Jack Knife Lucky Curve model ID help

Could someone help me with the model number of this pen.

 

The cap has an imprint that reads

Jack Knife 

Pat. June 4, 1912

 

The barrel imprint has a barely-visible Lucky Curve ribbon.

 

The nib's imprint reads

Parker

Lucky Curve

Pen 

5

 

The blind cap was missing when I purchased the pen, so the one in the photo is just an old one I had lying around for now...

I can't help with the ID, but wanted to say that is one gorgeous pen.

Don't feel bad. I'm old; I'm meh about most things.

It's in reasonably good shape for a 105-year old (assuming it's from 1919...) The curve of the Lucky Curve feed was sawed off long ago - although that's a common problem. The blind cap is both missing, so I'm on the lookout for a replacement... Worst, there's a wee bit missing off the tip of one of the tines, which is the biggest issue for me, since I actually use my pens...
