Esterbrook Dollar Pen Models

I now have a few Dollar Pens which I would like to restore and learn more about.  Three questions:

 

How can I tell what model I have H, A or B?   In the past I thought I saw article about how to ID a Dollar Pen but now can't find it in my search.  Should have bookmarked it!

Most of what I read recommends a size 16 sac for ALL Esterbrook's, but I thought I read somewhere that Dollar Pens may be different?

One of the cap bands is a little loose.  What can cause that and any advice on what to do about it?

Aside from the links provided, the easy (well, sort of) way to tell is this:
Full size pen - Dollar B

Slender pen - Dollar A

Short, slender pen - Dollar H

 

I find myself using #17 sacs for most J series pens, but the Dollar pens more often use a #18 or even a #20.
