To add to the PenBBS and Wing Sungs already suggested, I would mention the Caliarts Ego.

Piston Filler, about 2 mls, cap seals with an o-ring and is transparent, so any leaks are visible before you open it.

Comes with a Pilot 78g compatible nib, so if you aren't happy with the nib, you can replace it.

 

I have been using mine as an everyday carry for a couple of months now, and haven't had a skerrick of a problem.

+1 here, and I use a Ego-2 solid Sky Blue color vaersin as my EDC , actually just finish the ink yesterday. I have found that the solid color material more robust than the Demo. I have measure it to give tha the pen hold roughly 2.15 ml when fully filled

I've wondered if there is a way to adapt the cap of the ink bottle to other bottle types...

I imagine it would be possible to drill a hole in a cap and glue a shortened syringe needle in place; might not be as good as the original as there won't be a valve, but it could probably be made to work. There is one caveat however: the Pelikan Level bottle is a flexible plastic because you actually have to squeeze it to get the ink into the pen. So if you're going to modify a bottle, you want to pick a squeezable one.

I was thinking of mating a 5ml syringe to the cap of the bottle somehow. There's probably a way. My concern is that the Level bottle material will eventually give out. The cap should last longer though. So if you could find something replaceable that could stand in place of the bottle while keeping the cap, then you could have a long term solution. A syringe is easier to clean than an ink bottle...

 

I gave in and found one on eBay to order...but the seller is away and the order won't go through. I'm back to rethinking it.

True. But I've regretted not buying things that were unique and interesting too. It's only 35 bucks...hmmm. LOL.
